- To complete the waiver process, please ensure you have your 8-digit student ID number, date of birth, and a copy of your insurance card readily available.
- The waiver can only be submitted if the “Health Insurance Premium” charge appears on your student account and may require 48 hours from the posted date to be recognized in the UHC system. You may verify this by checking your Student Payment Portal and Account Activity Description and dates.

- Once approved, the “Health Insurance Premium Waiver” will offset the charge on your account. If this adjustment results in an overall credit balance, a refund will be issued.
- If it is determined that your insurance coverage does not meet the criteria for acceptable coverage, the waiver will be denied. In this case, you will have the opportunity to appeal the decision through United Healthcare.
- Applicable First-time students starting in the spring will incur a spring term "Health Insurance Premium" charge for the remainder of the academic year. Those applied the spring application are the only ones who can submit waiver during the spring term.
